Directed by MCG, the film explores a society where mandatory plastic surgery transforms sixteen-year-olds into "pretties," people who fit the culture's strict beauty ideals. At its core, "Uglies" delves into the dark side of a world obsessed with perfection, where the pursuit of beauty comes at the cost of individuality and free will.

The story follows Tally Youngblood, one of the last "uglies" waiting for her transformation. In this world, the operation is seen as an equalizing force that levels the playing field nearly identical appearances mean nearly identical survival rates. Upon reaching the Smoke, Tally discovers a community that embraces flaws and individuality, a stark contrast to the "perfect" world she comes from. She grows close to David, a member of the Smoke and the son of its founders, and learns about a dark secret behind the pretty surgery. While it may make one beautiful, the operation also implants lesions into the brain, making people docile and compliant. Tally's journey is filled with inner conflict. Initially sent to betray the Smoke, she finds herself questioning everything she once believed. Her relationship with David deepens, and she begins to see beauty beyond the physical. In a moment of defiance, Tally throws the locket into the fire, hoping to destroy it. Unfortunately, this action inadvertently triggers an automatic signal, leading to the invasion of the Smoke by the Specials. Devastated, Tally realizes that her actions have led to the destruction of the only place where she felt free.

The latter part of "Uglies" focuses on Tally’s desperate attempt to make amends. Alongside David, she manages to escape and hatch a plan to rescue others imprisoned by the Specials, including Shay and David's parents. Despite their best efforts, they arrive too late to save everyone. Az is dead, and Shay has been forcibly turned into a pretty. The escapees regroup in the ruins of an old city, where Maddy discovers a potential cure for the lesions. When Shay refuses the treatment, Tally, overwhelmed by guilt, decides to return to the city and undergo the surgery, volunteering herself as a test subject for the cure.

"Uglies" is a visual feast, contrasting the polished, utopian city of the pretties with the rugged, untamed landscapes of the Smoke.
